MARPE stands for Miniscrew-Assisted Rapid Palatal Expansion. It’s an advanced version of the classic childhood palatal expander, built for patients whose midpalatal suture, the seam running down the center of the roof of the mouth, has already started to fuse. That typically happens in the mid-to-late teen years, which is exactly why traditional, tooth-borne expanders stop working as well once a patient reaches that stage.
Instead of relying on the teeth to push the jaw apart, MARPE anchors directly into the bone of the palate using a small number of temporary miniscrews (also called temporary anchorage devices, or TADs). Because the force is applied to the bone itself rather than through the teeth, MARPE can achieve true skeletal expansion, actually widening the upper jaw, with far less unwanted tooth tipping than older tooth-borne designs.
This isn’t a fringe technique. It was first documented in a landmark 2010 case and has since been studied extensively in the peer-reviewed orthodontic literature, including systematic reviews and meta-analyses evaluating its safety, predictability, and long-term stability.

A widely cited review in the Dental Press Journal of Orthodontics (Suzuki et al., 2016) examined the biomechanics of the midpalatal suture and how miniscrew-anchored expanders are designed to achieve more purely orthopedic (skeletal, bone-level) movement compared with older expansion methods.
A 2021 systematic review and meta-analysis in the European Journal of Orthodontics evaluated MARPE’s efficacy specifically in late adolescents and adults, supporting it as an effective, minimally invasive alternative to surgically assisted expansion for many patients.
More recent scoping reviews (2023–2024) have continued to compare MARPE with traditional expanders like the Hyrax appliance, generally finding greater predictability and fewer dental side effects with miniscrew anchorage, while also noting that, as with any procedure involving miniscrews, a small proportion of patients can experience complications such as screw discomfort or minor tissue irritation, which is why careful case selection and an experienced provider matter.

Most patients feel pressure rather than pain, both during miniscrew placement (done with local anesthetic) and during the activation phase. Mild soreness for a day or two after activations is common and manageable with over-the-counter pain relief.
Active expansion typically takes several weeks, followed by a stabilization period where the appliance stays in place while new bone forms. Total timing varies by patient and is confirmed at your consultation.
No. MARPE is most valuable for older teens and adults whose midpalatal suture has begun to fuse, which is exactly the group traditional expanders struggle to help. Younger children with fully open sutures typically do well with a traditional expander instead.
A wider palate can increase nasal airway volume, and some patients report easier breathing after treatment. MARPE is not a standalone sleep apnea treatment, and we’ll refer you to an ENT or sleep specialist when airway concerns are significant.
