MARPE: Widen Your Jaw Without Surgery

You were told a narrow jaw meant surgery. It doesn’t have to.

If you or your teen has a narrow upper jaw, a crossbite, crowded teeth with no room to spare, or you’ve simply outgrown what a traditional expander can do, MARPE (Miniscrew-Assisted Rapid Palatal Expansion) may be the answer. It’s a research-backed, non-surgical way to widen the maxilla, even after the jaw has finished growing, using a custom, miniscrew-anchored appliance offered right here at Star Orthodontics.

What Is MARPE?

MARPE stands for Miniscrew-Assisted Rapid Palatal Expansion. It’s an advanced version of the classic childhood palatal expander, built for patients whose midpalatal suture, the seam running down the center of the roof of the mouth, has already started to fuse. That typically happens in the mid-to-late teen years, which is exactly why traditional, tooth-borne expanders stop working as well once a patient reaches that stage.

Instead of relying on the teeth to push the jaw apart, MARPE anchors directly into the bone of the palate using a small number of temporary miniscrews (also called temporary anchorage devices, or TADs). Because the force is applied to the bone itself rather than through the teeth, MARPE can achieve true skeletal expansion, actually widening the upper jaw, with far less unwanted tooth tipping than older tooth-borne designs.

This isn’t a fringe technique. It was first documented in a landmark 2010 case and has since been studied extensively in the peer-reviewed orthodontic literature, including systematic reviews and meta-analyses evaluating its safety, predictability, and long-term stability.

Who Is a Candidate for MARPE?

MARPE is most often recommended for older teens and adults, patients whose jaw growth has slowed or stopped, who are dealing with one or more of the following:

  • A narrow upper jaw or visibly constricted palate
  • A posterior crossbite (upper back teeth biting inside the lower teeth)
  • Crowded teeth with too little arch width to align them without extractions
  • A dentist or orthodontist has said your only option is SARPE (surgically assisted rapid palatal expansion) or full jaw surgery
  • Chronic mouth breathing, snoring, or other signs of a restricted nasal airway
  • Aligner or Invisalign treatment that isn’t tracking properly because the arch is too narrow

You may have heard of the term Airway Orthodontics, which is an approach to orthodontic care that considers how the teeth, jaws, and facial growth may affect breathing and airway function. It may incorporate expansion or growth-guidance techniques when appropriate, often in collaboration with other healthcare providers. MARPE can be an integral part of correcting your airway!

The best way to know if MARPE is right for you is a consultation with Dr. Robertson, who will review your growth stage, palate width, and airway with 3D imaging before recommending a treatment path.

Benefits of MARPE at Star Orthodontics

  • True Skeletal Expansion: Because force is applied through bone-anchored miniscrews rather than the teeth, MARPE widens the actual jaw structure, not just the dental arch, for a more stable, lasting result.
  • A Real Alternative to Surgery: For appropriately selected patients, MARPE can accomplish what once required surgically assisted expansion, without an operating room or general anesthesia.
  • Airway-Conscious Care: A wider palate means more room for the tongue and a more open nasal passage, which is why MARPE is increasingly discussed alongside snoring, mouth breathing, and airway health; we’ll coordinate with an ENT when appropriate.
  • Custom Design: Every MARPE appliance is fully custom to your palate. Dr. Robertson uses the iTero digital scanner and CBCT for precise, comfortable digital impressions, your device is designed and fitted perfectly to you.

The MARPE Treatment Journey

  • Consultation & 3D Imaging: We evaluate your palate width, bite, and airway, and confirm candidacy.
  • Custom Fabrication: Your MARPE appliance is designed and built specifically for you from your digital scans (iTero and CBCT.
  • Miniscrew Placement: A small number of miniscrews are gently placed in the palate, typically with local anesthetic; most patients describe pressure rather than pain.
  • Activation Phase: You’ll turn the appliance’s key on a schedule we set, gradually widening the palate over several weeks.
  • Stabilization: The appliance stays in place for a period after active expansion so new bone can consolidate in the widened suture.

Backed by Research

MARPE is one of the more thoroughly studied developments in modern orthodontics, and we want you to see the evidence for yourself, not just take our word for it:

A widely cited review in the Dental Press Journal of Orthodontics (Suzuki et al., 2016) examined the biomechanics of the midpalatal suture and how miniscrew-anchored expanders are designed to achieve more purely orthopedic (skeletal, bone-level) movement compared with older expansion methods.

A 2021 systematic review and meta-analysis in the European Journal of Orthodontics evaluated MARPE’s efficacy specifically in late adolescents and adults, supporting it as an effective, minimally invasive alternative to surgically assisted expansion for many patients.

More recent scoping reviews (2023–2024) have continued to compare MARPE with traditional expanders like the Hyrax appliance, generally finding greater predictability and fewer dental side effects with miniscrew anchorage, while also noting that, as with any procedure involving miniscrews, a small proportion of patients can experience complications such as screw discomfort or minor tissue irritation, which is why careful case selection and an experienced provider matter.

We believe in giving you the full, honest picture, the benefits and the realistic limitations, so you can make an informed decision together with Dr. Robertson.

Frequently Asked Questions About MARPE

Most patients feel pressure rather than pain, both during miniscrew placement (done with local anesthetic) and during the activation phase. Mild soreness for a day or two after activations is common and manageable with over-the-counter pain relief.

Active expansion typically takes several weeks, followed by a stabilization period where the appliance stays in place while new bone forms. Total timing varies by patient and is confirmed at your consultation.

No. MARPE is most valuable for older teens and adults whose midpalatal suture has begun to fuse, which is exactly the group traditional expanders struggle to help. Younger children with fully open sutures typically do well with a traditional expander instead.

A wider palate can increase nasal airway volume, and some patients report easier breathing after treatment. MARPE is not a standalone sleep apnea treatment, and we’ll refer you to an ENT or sleep specialist when airway concerns are significant.

Often, yes. MARPE creates the skeletal foundation and space; braces or Invisalign are typically used afterward to align the teeth within that newly widened arch.
